Liv is a Senior Primary Care Link Practitioner for the Aberdeen Links Programme. She is employed by Samh but work for Aberdeen Health & Social Care Partnership. They were set up in July 2018 and the team is made up of 4 Seniors and 19 Link Practitioner’s based in GP practices across the city.

The aim of the service is to support people to connect with their local community and overcome life’s challenges reducing their dependency on primary care health services.

They are now delivering a service within the Custody Suite working closely with the health team and Police Scotland and are just about to start a new service within Aberdeen Maternity Hospital ensuring new mothers have the relevant support/advice/information before they are discharged.

They have also set up a pilot project with one of the GP practices whereby through a link from their e consult form people can self-refer to the Links Service.

This became live on the 24th May 21 and we have already received 5 self referrals. The service is ever changing responding to the needs of local communities and the partnerships strategic plan.
